The Civilian Health and Medical Program of the Department of Veterans Affairs (CHAMPVA) is a health benefits program that shares the cost of certain healthcare services and supplies with eligible beneficiaries. This program is administered by the VA Health Administration Center (HAC) in Denver, Colorado. CHAMPVA eligible beneficiaries are the spouse or widow(er) and the dependant children of a veteran who:

• Rated permanently and totally disabled due to a service connected disability
• Rated permanently and totally disabled due to a service connected condition
  at the time of death
• Died of a service connected disability
• Died while on active duty
